In accordance with the 8 Edition of the Massachusetts State Building Code § 107.6.2, I hereby
certify that I have personally prepared the STRUCTURAL design drawing S -1 (dated October 24,
2011) for this project. To the best of my knowledge, this drawing conforms to the applicable
provisions of the Code and to accepted engineering practice.
I also hereby state that, to the best of my knowledge, the construction of the Structural Work for this
project was satisfactorily completed in accordance with that drawing, with the following exceptions:
1) In lieu of separate footings and piers, single -pour sonno- tube - formed piers were used with a
diameter equal to the required footing diameter. 2) In lieu of metal hangers for the two existing
timbers connecting to the new beam, additional PT framing was installed to carry those beam
reactions directly to the piers. These substitutions were reviewed and approved.
I also made one visit to the site to review the substantial completion of the structural work.
