application is administratively incomplete (see attached documentation).

Standard Permit Conditions:
1. All clearing, grading, drainage, construction, and development shall be conducted in
strict accordance with the following plans and information submitted with the application
and as amended by any conditions attached to this permit:

2. The Northampton Department of Public Works (DPW) or its authorized representative
shall be provided access to the property to conduct inspections before, during, and after
construction activities as necessary. The applicant shall arrange with the DPW to
schedule the following inspections: Initial Inspection, Erosion Control Inspection, Bury
Inspection, and Final Inspection. The DPW may also conduct random inspections to
ensure effective control of erosion and sedimentation during all phases of construction.
3. The applicant shall retain a Project Engineer (the design engineer or another
professional engineer of record in the Commonwealth of Massachusetts) to document the
construction of the proposed stormwater system and site development during all phases
of the project. The Project Engineer through routine inspections and surveys shall check
elevations, inverts, grading, layout, impervious areas and other critical aspects of the
stormwater system as they are being constructed during all phases of the project. All
proposed changes to the approved stormwater facilities and site development shall be
documented by the Project Engineer and submitted to the DPW for review and approval
by the DPW prior to construction. At the completion of the work, including construction
of all stormwater management facilities and landscaping, the applicant shall submit to the
DPW as -built plans stamped by a professional engineer of record in the Commonwealth
of Massachusetts and a statement from a professional engineer of record in the
Commonwealth of Massachusetts certifying that the materials and construction of the
stormwater management system was constructed in accordance with the approved plans
and functions as designed. The as -built plans must include surveyed information
documenting the as -built elevations, inverts, grading, layout, impervious areas and other
critical aspects of the stormwater management system.
4. Annual Reports that are stamped by a Registered Professional Engineer documenting
the actions completed as required by the Operation, Maintenance and Inspection
Agreement shall be submitted to the Northampton Department of Public Works by
October 111 of each year (see Specific Condition 1 below).
5. If construction of the proposed stormwater system has not started within five years of the
issue date of this permit, then the permit will expire and a new permit will be required.
Construction is defined as clearing, grading, excavation, filling, and installation of
drainage structures on the lots specified in this permit.

Specific Permit Conditions:
Prior to start of construction and issuance of any Building Permits, the applicant shall
record an executed Stormwater Operation, Maintenance, and Inspection Agreement that
is binding on all subsequent owners of land served by the private stormwater
management facility by recording it in the land records of the Registry of Deeds. The
maintenance schedule and responsibility for maintenance of all components of the
proposed stormwater management system must be adequately specified in Attachment A
to this Agreement. The draft Stormwater Facility Operations and Maintenance section of
the submitted document must be revised to be specific to this property to the satisfaction
of the DPW for use as Attachment A to the Agreement. The DPW can provide additional
guidance on required revisions to the maintenance schedule. The final agreement shall be
approved by the Northampton DPW and signed by the owner and the Mayor of
Northampton. The final agreement shall be recorded at the Registry of Deeds at the
owner's expense. Section 281-8 of the Northampton Stormwater Management Ordinance
provides additional guidance for the requirements of this agreement.
2. This project will be required to submit one or more Notice of Intent (NOI) to the U.S.
EPA for coverage under the NPDES Stormwater Construction General Permit. A copy of
the final NO[ and Stormwater Pollution Prevention Plan (SWPPP) shall be submitted to
the Northampton DPW for approval prior to the start of construction and prior to issuance
of a building permit for each phase of construction. The Erosion, Sedimentation, and
Pollution Prevention section of the submitted document is not adequate as a SWPPP for
the project.
3. Construction plans and specifications shall be submitted to the DPW for approval prior to
issuance of a building permit and prior to the start of construction.
4. Additional structures or practices are required to ensure improved water quality to the
City's stormwater system. The proposed Contech Cascade CS -5 Water Quality Unit has
only been certified at a 50% TSS removal rate by New Jersey
(https:lr'nj.gov/dep/stormwater/treatment.html) which is the only third -party certification
for this structure that was located by the DPW. Additional water quality measures are
necessary for the proposed stormwater management system. The existing parking lot and
driveway includes 14 catch basins with substandard sumps (0-1.8 feet) according to the
information provided in the Storm Drain Structures Data table on Sheet C-5. The
proposed plans specify the replacement of 6 of the existing catch basins with proposed
Storm Inlets that do not include sumps. The use of deep sump (4 foot), hooded catch
basins is considered standard engineering practice in parking lots and would be an
appropriate improvement in combination with the proposed treatment unit. The use of
vegetated treatment structures such as rain gardens and other low impact development
practices may also be considered to treat the stormwater. The applicant must provide
additional documentation of how the proposed stormwater management system will meet
80°a total suspended solids (TSS) rate of removal. Revised documentation of the TSS
removal rate and construction plans that include additional water quality structures must
be submitted to the DPW for review and approval prior to the start of construction.

5. Plan Sheet C-3 specifies Proposed 1,230 sf Pervious Pavement for the 7 parking spaces
along the driveway near Hatfield Street. Plan Sheet C-9 includes Detail #7 for Permeable
Interlocking Paver. The construction plans should clarify if pervious pavement or
permeable interlocking pavers are proposed for the 7 parking spaces. The specification
for of the materials and design of the permeable system must be revised in the
construction plans to meet the guidance provided in the Massachusetts Stormwater
Handbook which specifies additional layers of stone and does not specify geotextile filter
fabric.
6. Sheet C-5 includes a revision of the proposed stormwater system to specify one Cascade
CS -5 Water Quality Unit instead of the two Aqua -Swirl Water Quality Units and a
revised pipe design. The revision requires additional information and correction of an
error in the site plans. The proposed pipe size and material should be specified in all
locations. The pipe sections proposed to be abandoned should be labeled and additional
information provided for how abandonment will be completed. Particularly, the plan
must include information specifying how the existing structures (DMH -4 and DMH -3)
will be repaired or replaced. The DPW may require the replacement of DMH -3
depending on the condition of the structure. DMH -2 is identified as an existing structure
but there is no existing structure in this location. Please confirm that DHH-2 is a
proposed new manhole and specify this correctly in the plans. The construction plans
must be revised to address these issues.
