CONTACT THE SYSTEM DESIGNER AND CITY HEALTH DEPARTMENT PRIOR TO STARTING THE INSTALLATION SO THAT INSPECTION ARRANGEMENTS MAY BE MADE FOR THE SUBGRADE AND FINISHED SYSTEM. 2. A BENCHMARK IS TO BE TRANSFERRED BY BERKSHIRE DESIGN GROUP TO A LOCATION WITHIN 75’ OF PROPOSED SEPTIC SYSTEM PRIOR TO START OF CONSTRUCTION. 3. PROPERTY IS TO BE SUPPLIED BY MUNICIPAL WATER. NO KNOWN WELLS WITHIN 200 FEET OF PROPOSED SYSTEM. 4. BUILDING EXIT PIPES TO BE 4” DIA. SCH. 40 PVC OR ABS WITH A MINIMUM SLOPE OF 2% AND WITH NO BENDS GREATER THAN 22 DEGREES. SEWER LINE FROM 1 BEDROOM DWELLING TO HAVE A CLEANOUT TO GRADE AT MIDPOINT AS SHOWN ON PLAN. 5. PIPE FROM SEPTIC TANK TO D-BOX TO BE 4” DIAMETER SDR 35 WITH A MINIMUM 1% SLOPE. 6. LOCATION AND ELEVATION OF SEPTIC TANK MAY BE ADJUSTED IF MINIMUM SETBACK AND SLOPE REQUIREMENTS ARE MAINTAINED. 7. SEPTIC TANK AND D-BOX ARE TO BE PLACED ON A SIX-INCH BED OF STONE TO PREVENT SETTLING. SEPTIC TANK IS TO HAVE SCH 40 PVC INLET AND OUTLET TEES AND AN EFFLUENT FILTER (POLYLOK PL-68 OR SIMILAR) AS SHOWN IN PROFILE DETAIL. 8. REMOVE ALL TOPSOIL & SUBSOIL (APPROX. 24”) FROM DIRECTLY BENEATH LEACH FIELD AND FOR A DISTANCE OF FIVE FEET IN ALL DIRECTIONS, SCARIFY SUBGRADE AND REPLACE WITH CERTIFIED TITLE 5 SAND (AREA OF CUT AND FILL= 25’ x 40’). A SUBGRADE INSPECTION BY THE SYSTEM DESIGNER WILL BE REQUIRED PRIOR TO THE PLACEMENT OF ANY TITLE 5 SAND. 9. TO PREVENT SHORT CIRCUITING OF EFFLUENT THROUGH THE D-BOX A 4” TEE IS TO BE CEMENTED TO THE INLET AND THE FIRST TWO FEET OF EACH OUTLET PIPE IS TO BE LEVEL (OR SPEED LEVELERS MAY BE USED). 10. SET LEACH FIELD AT ELEVATION NOTED IN PROFILE, BACKFILL TO PROVIDE A MINIMUM 12” OF COVER AND CREATE A 2% CROWN TO DIVERT SURFACE RUNOFF AND PREVENT PONDING. 11. A 4” DIAMETER PERFORATED INSPECTION PORT WITH THREADED CAP IS TO BE SET IN THE LEACH FIELD AS SHOWN IN CROSS SECTION DETAIL. 12. ALL SYSTEM COMPONENTS TO BE MARKED WITH MAGNETIC TAPE OR REBAR PRIOR TO BACKFILLING. 13. ALL DISTURBED AREAS INCLUDING THE SOIL ABSORPTION SYSTEM TO BE LOAMED AND SEEDED (HAY MULCH MAY BE NEEDED ON SLOPES AND DURING WET TIMES OF YEAR). 14. SYSTEM IS NOT DESIGNED TO ACCOMMODATE A GARBAGE DISPOSAL. 15. AVOID DRIVING OVER THE SOIL ABSORPTION SYSTEM ONCE INSTALLED. 16. FIELD VERIFY DISTANCES TO PROPERTY LINES PRIOR TO ALL CONSTRUCTION AND OBSERVE LOCAL SETBACK REQUIREMENTS. 17. NOTIFY SYSTEM DESIGNER IMMEDIATELY OF ANY SITE CONDITIONS THAT ARE NOT NOTED OR INCONSISTENT WITH THOSE SHOWN ON PLAN. 18. CONTRACTOR IS RESPONSIBLE FOR ALL HORIZONTAL AND VERTICAL CONTROL. 19. ALL CONSTRUCTION TO BE IN ACCORDANCE WITH TITLE 5 OF THE STATE ENVIRONMENTAL CODE.
