A couple things; building permit sections 9 and 11 didn't get filled out; section 9 is architect, engineers and contractor information, and section 11 is owner's signature. You don't need architect or engineer signatures, just names and addresses. I attached the sections; you can email them back. We'll need to have information about who is doing the actual demo (including worker's comp, etc) and specific information about where the demo debris is going.
