[1] The property owner must sign the WPA Form 3.
[2] A wildlife habitat evaluation is required per 310 CMR 10.54(4)(a)5.
[3] The limit of Bordering Land Subject to Flooding (BLSF) should be shown on the plans. The Applicant should
address how the project complies with the performance standards for BLSF set forth in 310 CMR 10.57(4)(a).
[4] The legend on the plans is not comprehensive, making it difficult for this reviewer to evaluate the location of
delineated bank in relation to the proposed work.
[5] The Applicant should clarify the location of the upstream Aqua Barrier, as it does not appear to span the entire
width of the stream. The Applicant should clarify the location of the proposed dewatering basin within the stream
and why it is not located within an upland area.
[6] The Commission may wish to request a species list of proposed native plantings and may wish the Applicant to
clarify the proposed seed mixes.
