1) System Passes:
I have not found any information which indicates that any of the failure criteria described
in 310 CMR 15.303 or in 310 CMR 15.304 exist. Any failure criteria not evaluated are
indicated below.
Comments:
The septic system at a single family residence and home business (hair salon) was inspected and no
evidence of failure was found. System consists of one 1,500 gallon septic tank connected to a three
of distribution boxes and two 20' x 35' leachfields. Liquid levels in the septic tank and distribution
boxes were at the outlet inverts with no high staining. The 1,500 gallon septic tank and the
distribution boxes were in good condition. Use of a 4th bedroom as a hair salon must not be
continued.
2) System Conditionally Passes:
One or more system components as described in the “Conditional Pass” section need to be
replaced or repaired. The system, upon completion of the replacement or repair, as approved by
the Board of Health, will pass.
