2. a. Work Description (use additional paper and/or provide plan(s) of work, if necessary):
The paved pedestrian pathway at Fitzgerald Lake was installed 20+ years ago. The pavement
has exceeded its expected lifespan and is experiencing deterioration and root damage, making the
path difficult to access for people with mobility issues, creating tripping hazards, and in some places
crumbling. Resurfacing the 8’ wide path with a 1.5” overlay, from the North Farms Road parking area
to the wooden bridge/boardwalk intersection, a distance of approximately 1000’, is proposed. For the
most part the pavement is in good enough condition for an overlay. However, there are a few
locations where roots and other deterioration will likely require pavement removal and some root
pruning in order to repave. Also, there are a few areas that will need some shoulder material to
support the pavement edge. The overall footprint of the path, as well as drainage patterns, will
remain the same. Replacement of a pipe crossing with a bridge was permitted through a separate
Order of Conditions, but will be done concurrently with this project if funding is able to be secured.
The path travels near wetland resource areas, but no disturbance of jurisdictional areas, either
temporary or permanent, is proposed as part of this project.

I further certify that the property owner, if different from the applicant, and the appropriate DEP Regional
Office were sent a complete copy of this Request (including all appropriate documentation)
simultaneously with the submittal of this Request to the Conservation Commission.
Failure by the applicant to send copies in a timely manner may result in dismissal of the Request for
Determination of Applicability.
