The Housing Authority,through its undersigned representative hereby accepts from the Contractor;

The Contractor will be relieved of responsibility for performing further Work or supplying further materials,equipment,or other

• relieve the Contractor of liabilities with respect to any express warranties or guarantees required by the Contract.

Unit Prices—MDM has said that they needed to replace 41/z sheets of plywood. They will revise the
Record Drawings to reflect this and to show their locations, and they will provide photographic
documentation of the replaced plywood
