Cotton. The system is in compliance with 310 CMR 15.0.
The property was sold to Richard C. Bishop on December 19, 2016. At the time the Title 5 report was
considered a pass from the pending conditional pass.

pass based on repairs being made. Copy of Title 5 Inspection Form attached.
J.W. Cotton was hired by Upton Massasamont Realtors for Fannie Mae to repair the
septic system. Required repairs were completed on October 06,2016.
Upton Massasamont Realtors paid the invoice for said repairs being made.
December 19,2016 Property was sold to Richard C. Bishop per Hampshire County
Registry of Deeds recording.
