The Northampton Board of Health has inspected the premises at
18 Michelman Avenue, Northampton, MA (assessor's map 32C parcel 145-004.),
for compliance with Chapter II of the State Sanitary Code.
This letter will certify that the inspections revealed violations listed below,
which are serious enough as to endanger or materially impair the
health, safety, and well-being of the occupants
Under authority of Chapter III, Section 127 of the Massachusetts General
Laws, and Chapter II of the State Sanitary Code, you are hereby ordered to
make a good faith effort to violations
within FOURTEEN DAYS of the receipt of this order.

VIOLATION
Bathroom fiberglass tub enclosure in
Apt. #4 deteriorated. Stress fractures
in the floor of the unit allow water
seepage down through to flooring and
substructure.
Slate exterior roof and exterior brick
wall (left side of dwelling) are
deteriorated, allowing moisture
seepage into the dwelling units. loose
slates also create a safety hazard.
Many wall bricks are either missing or
badly deteriorated, allowing moisture
to pass through to the interior,
especially around window
casings/frames. The following areas
in Apt.#4 indicate visual evidence of
this:
(1) Dining area ceiling with
staininglchipping areas.
(2) Living room ceiling with severe
moisture damage and staining.
(3) Kitchen ceiling with staining.
(4) Outside wall dining area prime
window casings with evidence of
moisture see•a•eldama•e.
REMEDY
Repair I replace deteriorated tub
enclosure in an approved manner.
Take whatever action is required to
bring the roof and outer brick wall into
full code compliance with the housing
code. The roof and brick wall must
exclude wind, rain, snow so as to be
watertight and free from chronic
dampness.
Inspection of the premises was made on September 27, 1999 at
approximately 3:00 p.m.
NOTE: It has been brought to my attention that other tenants in this
complex have experienced similar problems with moisture seepage. Also,
several attempts to patch or repair the slate roofing have already been
failed.made and have it might
be prudent to considr
replacing the entire deteriorated roof. This may be the only option available
which will truly address all the code and safety issues noted.

conducting assessment activities. Based on the work performed, a Class A-2 Response Action
Outcome (RAO) has been filed with the DEP. It is the conclusion of our final report that no
