This letter will certify that the inspections revealed violations listed below, which are serious enough as to endanger or materially impair the health, safety, and well-being of the occupants. Under authority of Chapter III, Section 127 of the Massachusetts General Laws, and Chapter II of the State Sanitary Code,you are hereby ordered to make a good faith effort to correct the following violations within FOURTEEN DAYS of the receipt of this order. REGULATION VIOLATION REMEDY 410.250 Kitchen/dining area ceiling light fixture is very dim....lighting of this area inadequate. Repair/ replace bulb or fixture in a manner which will provide adequate lighting to this area. 410.351 (1) Back left bedroom-electrical coverplates broken or missing. (2) Kitchen cabinets badly deteriorated....Cabinet doors ill- fitted or missing. (1) Install approved coverplates where necessary. (2) Repair/replace kitchen cabinets in an approved manner. 410.500 & 410.501 Windows (1) & (2) Repair/replace all windows noted in an approved manner which will render them fully operational and weathertight. (3)& (4) Repair both doors in an approved manner. (5)through (9) Patch and repaint or otherwise repair all wall and ceiling surfaces noted. (1) Back right bedroom-Right lower window pane cracked. (2) Living room-Both front crank windows badly deteriorated. Cranking mechanisms busted rendering both windows inoperable. Doors (3) Interior cellar door is ill-fitted and not weathertight....Hinges loose. (4) Front prime entry door is not weathertight...air gaps noted between door and frame. Walls & Ceilings (5) Front bedroom walls deteriorated and need of repair minor holes and soiling noted. Inspection of the premises was made on August 24, 1998 at approximately 10:00 am.
