Nature of Incident: Dead body, 83 Massasoit St, concerns about sanitary
conditions, insect infestation
Immediate response actions: Ben arrived on scene, discussed issue with safety
officers on scene, saw picture of bathroom where body was found and discussed
action. Police concern that the house would be left unattended with a possible
nuisance condition existing. Ben said he would follow-up during the work week
but that there was no immediate threat to the community at the time.
Follow-up response actions: Ben Wood and Ed Smith entered the home on 6/2/11
and determined that no public health threat exists. See email dated 6/2/11 for
more details.
Lessons Learned: Need to develop a "dead-body" protocol for the police/fire
departments on when to notify the health department.

2011 at approximately 10:07 AM Ursula Leonhardt from Stickel, Frain & Llyod LLC arrived in
station and requested a well being check on Phyllis Mount.Lec nhardt advised Officer Dzialo that
at Mount's residents to drop off an Inheritance from Mount's at nt and was unable to get a hold of
