83 Police Report 2011 After-Hours and Unusual Incident Report Form Northampton Health Department Date of Incident: 5-28-11 Referring party: Police Department Responding person: Ben Wood Nature of Incident: Dead body, 83 Massasoit St, concerns about sanitary conditions, insect infestation Immediate response actions: Ben arrived on scene, discussed issue with safety officers on scene, saw picture of bathroom where body was found and discussed action. Police concern that the house would be left unattended with a possible nuisance condition existing. Ben said he would follow-up during the work week but that there was no immediate threat to the community at the time. Follow-up response actions: Ben Wood and Ed Smith entered the home on 6/2/11 and determined that no public health threat exists. See email dated 6/2/11 for more details. C Da55 _Modified ID: 179 --- Page:- 1 2011 at approximately 10:07 AM Ursula Leonhardt from Stickel, Frain & Llyod LLC arrived in station and requested a well being check on Phyllis Mount.Lec nhardt advised Officer Dzialo that at Mount's residents to drop off an Inheritance from Mount's at nt and was unable to get a hold of the re• ds dents they observed a Fed Ex package that they had sent ter weeks ago still on the tep. Person responded to Mount's residents, 83 Massasoit Stwith L r hahc door.the 3 aNl tha npton Fire re nbulance service. Officer Henderson was unable to get t Henderson and the fire department. While walking around the exterior of the two stmt/ home 1 se amount of flies on the interior of the windows. 1 also observed the mail to he stacked up in the the front porch. 1 observed the mail in the mail basket to have p)st marks dating back to April 28, located the bulk head in the rear of the home to be unlocker and gained entry into the home ind into to h check on Mount's well being. o iflies,observed ving and dead l lies. Once up stairs 1 observed several ig flies and several hundred dead flies through out the second tear. Officer Henderson opened the face ✓ and located a deceased female in the bathtub. At 10:42 AM I contacted the State 1 olive on call detective and spoke McMillan, Trooper McMillan later arrived on scene to assist wit]. the investigation. At 11:20 AM I Medical Examiner's Office who accepted the case. Dr. Parker wi h the Medical Examiner's Office case and they advised me that someone from their office would 1 responding to remove the body )ody world he transported to the Medical Examiners Office in Bos on for an autopsy. sski arrived on scene and photographed the interior and exterior of the home. Inside of the home obinson and I were able to located two prescription bottles in the kitchen. One of the bottles was was prescribed by Dr.Paula Moore. The second bottle was for Pexc fenadine prescribed by Dr. Adam )etective Robinson contacted Dr. Carretson by telephone. Dr. Gam tson advised that be was Mount's physician, but stated that be had only seen her three times from AT ril of 2009 through May of 2010. m stated that he had only seen Mount for acute problems (Tick bi e, Scabies). Dr. Garretson stated had told him that she had Viral encephalitis, but he stated that he dic not have any was evidence or r hi history have this. Mount also told him that she had a disorganized though process, and y schizophrenic.
