S T.JOYCE,Chairman IE BLRES, M.D. RAE/R.PARSONS ER J..MeERLAIN,Health Agent December 4, 1995 C. 1-1" OF VOA'CriGi'4.'U^: MASSACHUSETTS 01060 OFFICE OF THE BOARD OF HEALTH Mrs. Carol S. Flynn do Hampshire County Long Term Care Facility River Road Leeds, MA 01053 RE: 134 Spruce Hill Avenue Dear Mrs. Flynn: 210 MAIN STREET 01060 (413)586-6950 Ext.213 On December 2, 1995, in response to a request from the Northampton Police Department, I inspected the dwelling at 134 Spruce Hill Avenue owned by you. That inspection revealed that the dwelling at 134 Spruce Hill Avenue had some very serious sanitation problems, as follows: 1. a very large accumulation of trash and refuse throughout the dwelling. The material is piled high on every flat surface, including tables, counters, chairs, beds. In addition, the entire floor was littered with mounds of refuse which made waking difficult, 2. plumbing fixtures were plugged and inoperable. These conditions constitute violation of Chapter II, Section 410.351(A) and 410.602(B) of the State Sanitary Code 105 CMR 410.000. It should be noted that, while work had begun on the cleanup of the dwelling, the conditions at the time of inspection rendered the dwelling unfit for habitation. Therefore, under the authority of Chapter III, Section 127A of the Massachusetts General Law, the Northampton Board of Health hereby orders that the dwellling at 134 Spruce Hill Avenue remain vacant until the cleanup and disposal of refuse is completed and the plumbing fixtures are all functional. Please contact the Board of Health Office prior to reoccupying the dwelling so that a reinspection can be conducted. Thank you for your anticipated cooperation in this matter. Please feel free to contact the Board of Health Office with any questions concerning this matter. Very truly yours, Peter J. McErlain Health Agent PJMcE/cdh cc: Stephen Flynn H.C. 66, P.O.
