The inspection was conducted by Charles Rudnik accompanied by Captain Burt. It is important that strict attention be paid to the chemical sanitizer concentrations. Concentrations which are too high could be toxic. In accordance with Section 451.404 of the previously mentioned regulations please indicate next to each entry on the enclosed Report of Condition, a plan of correction. Clerk, Massachusetts Senate Clerk, Massachusetts House of Representatives Board of Health Food and Drug Division kg. 2 Hampshire re Kitchen (con't) HOMES: 15 Minutes before conducting an inspection of the kitchen, the electric breaker controlling the supply of electricity to the dairy walk-in cooler, the preparation walk-in cooler, the walk-in freezer, a hot holding unit, an upright cooler and the walk-in freezer located in the warehouse, tripped: rusing a loss of power to this equipment. Electricians were immediately notified and corrective work started in several minutes. Food in hot holding unit was immediately transferred to working hot holding units at the rear of the kitchen. At 2:45 P.M. staff checked the internal temperatures of the food products in both walk-ins and checked the air temperature in the walk-in freezers. The following temperature were recorded: Dairy Walk-in - milk product recorded at 37°F. "Prep" Walk-in - sliced meat recorded as 34°F. - Log of meat recorded as 36°F. Walk-In Freezer - Air temperature recorded as 21°F. , product frozen solid Warehouse Walk-In Freezer - Outside gage - read - 4°F. below zero, product frozen solid At 3:50 Y.M. the electrical problem was corrected by replacing a fuse. The temperatures of the walk-in freezers and coolers were checked again at 4:00 P.M. The following results were obtained: "Prep" Walk-In - 38°F. recorded for sliced meat and log of meat Walk,In Freezer - Air temperature recorded as 10°F. , product was frozen solid Warehouse Walk-In Freezer - Air temperature - 0°F. , product frozen solid Electricity to the hot holding unit and to the upright cooler, was also restored at 3:50 P.M. The inspection was conducted by Charles Rudnik accapanied by Lt. Hodgen. The hot water within mast hand wash sinks is delivered at less than the required 110° - 130°F. The hot water, however, at the hand wash sink in the kitchen of the nodular unit is 152°F. This presents a severe scald injury. There is no exhaust ventilation within the shower forwarding a copy of aslnoted in the previous report and was responded by flume Reinhardt Assriates dated August 29, 1986. That letter, however, did not address the issue. Apparently the statement ' Circulation of outside air or recirculated filtered air per human occupant is in the facility" logs intended to do so. We continue to be confused as to the meaning of that statement as it does not address the issue of no exhaust ventilation serving the shower facilities. This structure was built under the supervision of the Massachusetts Division of Capital Planning and Operation. We have had semi-annual inspections by your agency since our opening in 1984 and this issue did not surface until this past year. It appears to me that either someone has been missing something or someone is making a non-issue an issue. In the spirit of cooperation,I contacted the building architect Reinhardt Associates to explain the situation and the construction. It would seem reasonable to me that you could resolve any problem with Reinhardt, D.C.P.O. or someone that was involved with the design and construction of this building. I am simply trying to administer a government function in a building that was constructed under the supervision of one of your sister State-agencies. The Work Release Coordinator has made note of this in the Pass Down Log previously. **590.004 B (3) The freezer had been turned up. corrected at inspection. **590.004 B (1) The inaccurate thermometer was actually broken, and replaced. 451.111 Soap and paper towels provided. **451.200 The standard listed pertains to Minimum Sanitation Standards for Food Establishments, and the condition cited by the inspector refers to living units. 3 451.140 Due to the continued confusion in regards to Adequate Ventilation in the shower stalls: This was reported by the Inspector in the previous Inspection of 12/9/93. However, the standard listed was 451.141 which pertains to screens, not showers, so that was what was responded to in the Plan of Correction. We then received a letter on 1/18/94 requesting clarification on our response to the ventilation issue. The response that we gave was the same response that we received from the architect. The next communication received was 2/17/94 and raised concern as to the statements made by the architectural firm. We have no control on the statements made by the architectural firm. The inspection report received 10/12/94 now has the correct standard listed for adequate ventilation. However, the standard does not reflect the showers stalls, that we have at this facility. The standard states "Each inmate shall have adequate ventilation at all times. Each facility shall have ventilation of each cell and each habitable or occupiable space, bathroom, and toilet compartment in compliance with 780 CMR, State Building Code. Natural ventilation through a window in each cell is recommended." All of the criteria of this standard is met by the facility, and if there is any question of compliance with 780 CMR, then it should be addressed to DCPO, who was responsible for overseeing of construction of the facility. The Day Room is properly ventilated and hence with the shower stall open to the Day Room becomes a part of the Day Room space and does not require separate ventilation provisions. The inspection was conducted by Charles Rudnick accompanied by Lt . Hodgen. The inspection revealed that improvements have been made since the previous inspection. It is our understanding that the ventilation problem in the shower areas is being investigated by DCPO. Problems were noted in several areas of the institution with inadequate hot water temperatures in some instances the temperature was as low as 82°F. The appropriate temperature for showers and handwash sinks is between 110°F and 180°F. Inadequate temperatures were also noted at the mechanical dishwasher in the modular unit . Until proper temperatures can be reached, manual sanitation practices or single services must be used. In accordance with Section 451.404 . of the previously mentioned regulations please indicate next to each entry on the enclosed Report of Condition, a plan of correction. The report, Project No. DCP909-HD3 Job I, was prepared by ASE Corporation of Quincy MA for the DCPO Office of Project Management. The standard that is listed is incorrect. The correct standard for the violation is 451.383. The violation was corrected at the time of the inspection. The inspection was made by Charles Rudnik, R.S., of this Department, accompanied by Lieutenant Hodgen. The hot water temperature throughout the building is uneven providing temperatures both above and below the required range of 110 to 130 degrees F. Although the room temperatures were adequate at the time of the inspection, several inmates complained that the rooms are cold at night. The temperatures must be maintained at least at 64 degrees F. It was noted that a product known as Maintainer DIS is being used on the floors. The Material Safety Data Sheet lists this material as having a high health hazard. It is a severe eye irritant as well as an initant to the skin and through inhalation. It is suggested that you may wish to explore other less toxic materials. In the interim,the user of this material should be provided with protective eyeware and gloves. Modular Units Kitchen .200 Buckled floor tiles under coffee machine and dishwashing area.
