The Northampton Board of Health has inspected the premises at
Coolidge Park , 50 Union St, Northampton, MA (assessor's map 32A parcel 58 .),
for compliance with Chapter II of the State Sanitary Code.
This letter will certify that the inspections revealed violations listed below,
which are serious enough as to endanger or materially impair the
health, safety, and well-being of the occupants.
Under authority of Chapter III, Section 127 of the Massachusetts General
Laws, and Chapter II of the State Sanitary Code,you are hereby ordered to
make a good faith effort to correct the following violations
within seven days* of the receipt of this order.
UTATION
VIOLATION
410.500 Mold/ Mildew growth observed in
basement storage. Problem may be
due to chronic dampness and/or the
result of a recent water heater leak.
TOTE: A reinspection of the basement area, conducted on 10115/98 at the request of
operty Manager Chaput, revealed that the moldlmildew problem had already been
caned up.
DTE 2: It is strongly recommended that children not have access to the storage and
at toys and equipment used by children not be stored in the basement to prevent future
mtamination by moldlmildew.
If you have any questions regarding this abatement order contact the Board
of Health office.
This inspection report is signed and certified under the pains and penalties
of perjury.

Dear Mr. Armour:
At your request,on November 2, 1998, I conducted a follow-up inspection of the cellar in
your building at 50 Union Street. That re-inspection revealed that the building management
had taken no additional action(such as the installation of ventilation or dehumidifying
equipment),to control dampness in the cellar area.
It has been previously noted that the original mold&mildew growth was removed from the
cellar walls.
