This letter will certify that the inspections revealed violations listed below, which are serious enough as to endanger or materially impair the health, safety, and well-being of the occupants Under authority of Chapter III, Section 127 of the Massachusetts General Laws, and Chapter II of the State Sanitary Code, you are hereby ordered to make a good faith effort to correct the following violations within FOURTEEN DAYS of the receipt of this order. ATION VIOLATION REMEDY (1) Kitchen gas stove with one front burner not operational. (2) 2"''floor front bedroom outer wall baseboard electrical outlet not secured to the wall. (3) 2' floor front bedroom inner wall baseboad electrical outlet not secured to the wall. (4) Ceiling Light switch operation defective; electrical wiring exposed. (1) Repair/replace malfunctioning burner. (2) Secure loose electrical oulet in an approved manner. (3) Secure loose electrical oulet in an approved manner. (4) Repair/replace ceiling light fixture/switch in an approved manner. I, I, & Kitchen All window, window components, (1) Side prime window ill-fitted, deteriorated, and not weathertight...Window and frame with rotted components. (2) Front prime window not lockable and screening is torn. Pantry locks, and window screens All prime windows must be weathertight,fully lockable, open and close easily; remain in position when let go, have all panes uncracked and intact; free of rotting components, including all sills and frames. Screening must be tight-fitting and free of holes. (Expandable screens to not meet code requirements.) (3) Prime pantry window ill-fitted, deteriorated, and not weathertight; window is not lockable; screening is torn. Bathroom (4) Prime window ill-filled, deteriorated, and not weathertight...Window and frame with cracked and peeling surfaces; window screen missing. (5) Bathroom walls with cracking surfaces. (6) Bathroom flooring tiles beneath the tub are missing along the floor/wall juncture....not moisture tight. Living Room (7) Side prime window ill-fitted, deteriorated, and not weathertight; window and frame with rotted, cracking components- window will not lock. (8) Back prime window(to porch) deteriorated and not weathertight; Panes loose and separating; Window will not lock. (9) Peeling wall surface in living room near the entryway to kitchen. 2nd Floor Front Bedroom (10) Front prime window ill-fitted, deteriorated, and not weathertight; rotting components; screen ill-fitted; window will not lock. (11) Some wooden flooring boards are loose and sagging; some with split wood. 2nd Floor Back Bedroom (12) Prime window with cracked window pane; window sill rotted; window will not lock. 1st Floor Bedroom (13) Side prime window ill-fitted, deteriorated, and not weathertight; rotted components' window will not lock. (14) Back prime window ill-fitted, deteriorated, and not weathertight; rotted components; 1 cracked window pane; window will not lock properly. (5) Re-finish bathroom walls in an approved manner (6) Repair bathroom flooring in an approved manner. (9) Repair peeling wall in an approved manner. (11) Repair loose and damaged flooring boards in an approved manner. Large accumulation of junk in cellar in vicinity of gas burner left by previous occupants and never removed by owner prior to re-leasing of apartment. Remove all accumulated junk front cellar and properly dispose of it through approved means. Inspection of the premises was made on August 1, 1997 at approximately 10:30 am.
