B, Northampton, MA (assessor's map 31B parcel 029 .), for compliance with Chapter II of the State Sanitary Code. This letter will certify that the inspections revealed violations listed below, which are serious enough as to endanger or materially impair the health, safety, and well-being of the occupants. Under authority of Chapter III, Section 127 of the Massachusetts General Laws, and Chapter II of the State Sanitary Code, you are hereby ordered to make a good faith effort to correct the following violations within FOURTEEN DAYS of the receipt of this order. .ATION 1 VIOLATION 0 12 (1) Back entry hallway wall light fixture is not operational. (2) Bathroom shower head assembly leaking profusely. Side entrance prime exterior door must be capable of being reasonably secured. Tenants living off this entrance should be provided access keys. Apartment fire alarm lacks a suitable cover; is not operational at this time. (1) Chronic moisture seepage through the kitchen flooring tiles; moisture mixing with tile adhesive and seeping up through seams. (2) Concrete shower floor is absorbing moisture, causing chronic mildew buildup in shower and an accumulation of additional moisture which is slowly deteriorating wooden structural components near the shower base. (3) Lower section of the bathroom closet must either be refinished and tightly sealed with a non-absorbent material, or permanently removed and remaining exposed wall surfaces properly covered. REMEDY (1) Repair light fixture so as to be fully operational. (2) Repairlreplace shower head assembly so as to eliminate all leaks. Provide a keyed locking mechanism for this entrance. Provide tenants with access keys. Repair/replace fire alarm in an approved manner. (1) Take whatever action is necessary to correct this ongoing problem on a permanent basis. (Recommend raising floor substructure slightly.) (2) Repairlreplace shower floor in a manner which will alleviate this chronic moisture problem. (3) Take action to correct this problem in an approved manner. 8, Jed) (4) Left bedroom: Large accumulation of paint chips behind the baseboard heater. (5) Living room closet with two holes along the lower section of back wall which appear to provide access to plumbing line. (4) Cleanup all chips/debris behind this baseboard heater. (5) Tightly seal off opening in this closet wall so as to eliminate moisture, dampness and insect insect access.
