On July 17. 2012, I visited the above address at the request of Mr. Michael Pancione from
Pancione Associates Regional Realty, LLC. Mike informed me that he would be making some
foundation repairs to the top of a foundation wall located under a gable wall at the north side of this
structure.
The structure at the above address is a four unit R-2 use group apartment house. The cubic
area of this structure is likely over 30.000 cubic feet and will require controlled construction reports
while the proposed foundation repairs arc being made.
The foundation wall that Mike intends to repair consists of a multi-wyth brick foundation
wall from the existing wood sill above to slightly below grade. This brick wall cap bears upon a dry-
stacked stone foundation wall that starts slightly below grade and ends at an unknown depth below
grade. The wall to be repaired is located at a small crawl space that juts off of a full basement. The
repair is being made because the existing wall to be repaired has bowed inwards at the brick and
field stone interface located slightly below grade. The brick foundation wall cap.nor the dry stacked
foundation wall below is no longer plumb.
Mike intends to install temporary needle beams to support the existing main floor and gable
wall above the foundation wall in question. This will allow him to rebuild and repair the foundation
as required to return this foundation wall to a plumb location beneath the wall above.
Mike asked me to design a new main floor dropped beam to install near the foundation wall
repair location. There is currently a wood dropped beam that supports the main floor joists at the full
basement/crawlspace location. This wood beam is supported by masonry counterforts at the beam
ends and a wood column at the beam mid-span. The existing foundation supports belwo the existing
dropped wood beam does not look to he in great condition.
A new main floor steel beam will be installed south of the existing wood beam. lhis new
beam will support the same joists as the existing wood beam. The existing beam and the new beam
will only support a small area of the main floor and does not support and additional load from above.

The new beam shall be a W8x15 shape of ASTM A992 grade steel. This new steel beam
shall be two spans continuous and supported by three 3 %i' nominal schedule 40 pipe columns.
These new columns shall be located at the beam ends and one at mid-span. The new pipe columns
shall have 3/8"thick cap and base plates welded to the schedule 40 pipe columns with continuous
3/16 Fillet welds. The 3/8" thick columns cap plates shall be bolted to the bottom flange of the new
steel beam with four /2" diameter thru bolts, ASTM grade A307 at each location. The bottom of
these columns shall be supported by new 1'-8" x 1'-8-x 10"thick unreinforced concrete pad
footings.The new columns shall be attached to the new footings with four '/" diameter expansion
anchors.
