based on my training and experience in the proper function and maintenance of on-site sewage disposal

The Northampton Board of Health is in receipt of a septic system inspection report(from Inspector
Tom Leue),which indicates that, the septic system at 64 Chapel Street fails jg meet the reouirements
of Section 15.303 of CMR 15.000, State Sanitary Code, Title 5.
Because the dwelling has direct access to the city sewer line on Grove Street the Board of Health
requires that the dwelling be connected to the city sewer system.
This letter will confirm that the septic system at 64 Chapel Street has failed and that the now vacant
dwelling be connected to the sewer line prior to being reoccupied.
Ms. Muralee Salvatore, representing the Sanderson Estate(property owners)will be contacting you
regarding a permit to access the Grove Street sewer line.

RE: Sewage Disposal System Inspection

The Northampton Board of Health is in receipt of a report on the Subsurface Sewage Disposal System Inspection
conducted by Tom Leue at 64 Chapel St.,Northampton on April 17,2001.That inspection report indicates that the
subsurface sewage disposal system at that address fails to protect the public health and the environment as
defined in Section 15.303 of CMR 15.000,State Environmental Code,Title 5.
Therefore, in accordance with the provisions of 310 CMR 15.000 of the State Environmental Code,Title 5,and under
authority of Massachusetts General Laws, Chapter 2IA, Section 13,the Sanderson Estate(or the subsequent owners of
the property )is hereby ordered to repair the subsurface sewage disposal system at 64 Chapel St..,within two years of
the date of the original inspection,(by 4/17/03). If further degradation of your sewage disposal system occurs(e.g.
sewage flowing to the surface of the ground),the repairs will be required sooner.
All work to repair/upgrade the subsurface sewage disposal system must be performed by a licensed sewage disposal
system installer, in accordance with the requirements of 310 CMR 15.000, and with plans approved by the Northampton
Board of Health. Note: if connection to city sewer line is possible it may be required in place of repair
Please be advised that you are entitled to a hearing on this order to upgrade your subsurface sewage disposal system,
provided that you file a written petition requesting such a hearing in the Board of health office within seven(7) days of
the receipt of this notice.
