Administration:
The Engineer visited the site to inspect the concrete floor slab and related concrete accessories. This will
be the final inspection and report attesting that the foundation system is complete and thereby concludes
this Engineers area of responsibility.
Field Observations:
The Engineer noted a layer of poly vapor barrier
separating the 4' slab from the grade. A layer of
6x6 welded wire mesh was in place and was being
lifted off the grade as the concrete floor was being
poured,thus insuring that the mesh was situated
within the concrete slab. The hairpin at column
line E-4 was correctly placed and the area
excavated to allow ample concrete coverage.

George Bucala,the project manager, informed the Engineer that the design strength of the concrete for the
floor slab used was 4000 psi. In addition,a curing membrane will be laid on top of the new slab to provide
a controlled hydration period and to protect the slab from the elements. The Engineer was also informed
that control joints will be sawcut into the slab when the concrete has hardened enough to do so.
Miscellaneous:
The foundation system and floor slab will be finished upon the completion of the concrete slab. The
foundation was successfully constructed according to the plans and specifications meeting the requirements
of the Massachusetts State Building Code as well as the requirements of the American Concrete Institute
Code Requirements for Structural Concrete.This will be the final report for this foundation system.

Upon this supplemental inspection, the Engineer noted that all exit and emergency
lighting on the first floor of the building was installed. The Engineer checked the
operation of the emergency lights and found that two heads on separate fixtures failed to
light. The project manager was informed and the Engineer was assured that the lights
will be operational prior to the buildings occupancy. It was noted that fire detectors and
pull boxes were also in place.
The Engineer is satisfied that the new addition was built according to the plans as
submitted to the building department and is adequately prepared for the Owners use to
allow construction to continue in the existing building.

The sprinkler system is complete. The Engineered verified the number and location of
the sprinkler heads including those on the second floor. The fire department connection
is complete and it was noted that the flow sensor was installed.
Supply and return vents for the HVAC system was completely installed with some
variation in their locations; most notably in the shipping and receiving area where the
supply and return was separated considerably more than shown on the plan to prevent a
short circuit in air flow.
The 110v electrical outlets and some switches were installed. The 2x4 troffer lights were
in place but some locations were altered from the plan. Emergency lighting was installed
in the Assembly and Quality Control areas of the building while junction boxes were
noted in locations where exit and emergency/exit lights are to be installed; specifically at
the top of the exit doors and the top of the door into the existing office. The Engineer
noted boxes and wiring for fire detectors in Shipping and Receiving, Assembly, Stock
Room, Cubical area and two in the Quality Control area. Junction boxes for the pull box
alarms were installed in both stairways in the new building.
The existing wall was removed to open the Shipping and Receiving area to the existing
building as shown on the plan. Demolition in the existing building has begun.

Miscellaneous
The Engineer was informed that the Owner is seeking a temporary occupancy permit to
allow the use of the new space while the existing building is being renovated as shown on
the plans. The Engineer performed a supplemental inspection on July 30 to insure the
installation of life safety devices prior to such occupancy.

The Engineer visited the site to record the progression of the building's construction
including interior finishes and the installation of life safety devices.
Progress/Inspection
Wall and floor finishes were being applied at the time of the visit. The tiles on the
lavatory floors were installed while the VCT flooring was underway in the Stock Room.
Some ceiling tiles were in place. All of the hollow metal door frames are installed and
painted with wood doors in place in the Stock Room, new offices, the new 3' wide
opening into the existing building and the sprinkler room. The Engineer verified that the
hollow metal door assemblies installed in the stairway walls were 1 1/2 hour fire rated.

Ductwork for the HVAC system was installed according to plans obtained from
McCormick Allum with some exception(see"Variations from submitted plans"). Most
of the supply and return vents are installed. The Engineer noted drops for a vent system
coinciding with the cubical layout in the northwest portion of the new building.
Rough plumbing for the fixtures shown on the plans was complete including supply and
waste/vent pipes for the two sinks in the new addition and the two new lavatories in the
existing building. The copper vent in the plumbing wall was in place but was not yet
projected through the roof. Stubs for hot and cold water supplies were installed in the
west wall of the Quality Control area of the new building. In addition, a humidifier was
installed and plumbed in the"Assembly" area.
The sprinkler system in the new building is about 95% complete. Most of the sprinkler
heads were noted and verified as to number and location shown on a plan provided by
IIFP Corporation with some exception(see"Variations from submitted plans"). In
locations where heads were not yet installed, unions indicating such heads were observed.
The sprinkler service entrance in connected to the interior sprinkler piping system. The
fire department connection in not yet complete.
Panelboard boxes for two subpanels are installed in the Quality Control area of the new
addition. Boxes for switches, outlets and emergency lighting were also installed.
Variations from submitted plans
Two new offices were added along the east wall of the new addition. This effected a
change in the locations of some HVAC and sprinkler components. A new computer
room was added in the existing building where a closet had originally been shown. The
two new lavatories were moved west to accommodate the new room. It was noted that
half walls were added in Quality Control.
Miscellaneous
The Engineer was unable to view the second floor as the concrete steps were freshly
poured at time of the visit.
The new addition is nearly complete and ready for occupancy. The Engineer will
perform a final inspection and report upon notification that life safety components are
installed and operational.

the new building are installed but the tiles are not installed due to continued work above
the ceiling line.

WHEREAS Condition #1 of a Site Plan Decision recorded in the Hampshire Registry of
Deeds in Book 9291, Page 7 of the Northampton Planning Board requires Microcal, LLC to
grant the City an easement for the drainage easement shown on the Site Plan and prepare and file
a maintenance plan for the property owned by Microcal, LLC by deed dated December 14 1999
and recorded in the Hampshire Registry of Deeds in Book 5850, Page 280 and affected by the
Site Plan Decision; now

C. The requested use will have a positive effect on the relationship of structures and open spaces to the natural landscape, existing
buildings and other community assets In the area. infill in the Industrial park to maximize land area is a goal of the plan and there is
enough open space on this site to remain between buildings. The rear of the site abuts 1-91 and will not have an impact.The
Conservation Commission has approved the plan with mitigation for wetlands and butler Impacts.
D. The requested use will not overload the City's resources including the effect on the City s water supply and distribution system,
sanitaryand storm sewage collection and treatment systems,fire protection,streets and schools.AM stormwater systems will comply
with DEP standards. Roof drainage 071 be directed to a planted rain garden.Site drainage will flow around the perimeter of the site In a
public system. The applicant will rebuild and redirect the public stormdrain system in accordance with DPW guidelines.

1.Prior to issuance of a BUILDING PERMIT,the following items must be completed.
a. The applicant must record easement and maintenance plans,with language approved by
Department of Public Works,for the drainage easement area indicating that the applicant will be
responsible for replacing all materials including landscaping and parking infrastructure if damaged
from work performed by Department of Public Works within the easement area.
b.An operations and maintenance plan for the stormwater system must be recorded.
2.Perforated concrete,as demonstrated in the hearing,shall line the relocated Swale in coordination
with Department of Public Works.
3.An annual report(December of each year)on the use of the ride share program shall be submitted
to Office of Planning and Development.
4.As offered by the applicant,the ride share incentive program shall be open to any employee who
opts to come to work without a personal vehicle.This could include sharing rides with coworkers,
other employees in the Industrial Park,riding the bus,walking,biking,etc.
5. Prior to issuance of a certificate of occupancy,the applicant shall construct the sidewalk as
proposed along Bates Street in consultation with Department of Public Works.

F. Appeals
The applicant, the owner, any person aggrieved by this Order, any owner of land abutting the land subject
to this Order, or any ten residents of the city or town in which such land is located, are hereby notified of
their right to request the appropriate DEP Regional Office to issue a Superseding Order of Conditions.
The request must be made by certified mail or hand delivery to the Department, with the appropriate filing
fee and a completed Request of Departmental Action Fee Transmittal Form, as provided in 310 CMR
10.03(7)within ten business days from the date of issuance of this Order. A copy of the request shall at
the same time be sent by certified mail or hand delivery to the Conservation Commission and to the
applicant, if he/she is not the appellant. Any appellants seeking to appeal the Department's Superseding
Order associated with this appeal will be required to demonstrate prior participation in the review of this project.
Previous participation in the permit proceeding means the submission of written information to the
Conservation Commission prior to the close of the,public hearing, requesting a Superseding Order or
Determination, or providing written information to the Department prior to issuance of a Superseding Order
or Determination.
The request shall state clearly and concisely the objections to the Order which is being appealed and how
the Order does not contribute to the protection of the interests identified in the Massachusetts Wetlands
Protection Act, (M.G.L. c. 131, § 40) and is inconsistent with the wetlands regulations (310 CMR 10.00).
To the extent that the Order is based on a municipal ordinance or bylaw, and not on the Massachusetts

33, As offered by the applicant for additional wetland mitigation, the applicant provides
fifteen thousand dollars ($15,000) to the Conservation Fund of the City of Northampton
Conservation Commission for wetland replication, enhancement or preservation.
34. Applicant either creates the replication area shown on the plan, creates an area of equal
size replication area within the same watershed (to be approved by the Northampton
Conservation Commission) or provides the City of Northampton with an additional
fifteen thousand dollars ($15,000) toward a wetland replication project within the
watershed.
27• Soils exposed for periods greater than two months shall be stabilized with erosion control
blankets and netting, a covering of straw mulch, or a temporary cover of rye or other
grass to prevent erosion and sedimentation. Drainage ditches shall be stabilized and
seeded with a native perennial grass mixture. Any stabilization materials such as jute
netting shall be firmly anchored to prevent them from being washed from slopes by rain
or flooding. Preference should be given to biodegradable materials.
28. All disturbed areas shall be graded, loaned and seeded, or stabilized with erosion control
blankets or netting, and a covering of straw mulch prior to November 30, of each year.
No disturbed areas or stockpiled materials will be left unprotected or without erosion
control after this date.
29. No disposal of soils or other materials shall be allowed within: a 100-year floodplain; 40
feet of the 100-year floodplain elevation; any wetland; or any area within 100-feet of a
wetland, unless such areas are specifically approved by the Commission, in accordance
with 310 CMR 10.00, and City of Northampton Ordinances - Chapter 24
30. Ongoing conditions that shall not expire with the issuance of a Certificate of Compliance
are as follows: N/A
31. Upon completion of the work covered by this Order, the applicant shall submit an as-built
plan, signed and stamped by a registered professional engineer or land surveyor, together
with a written request for a Certificate of Compliance.
The plan and written request shall specify any ways that the completed project differs
from the plans referenced in the Order. The as-built plan shall include, at a minimum, and
as applicable to the project: elevations of all pipe inverts and outlets, pipe sizes,
materials, and slopes; all other drainage structures; limits of clearing, grading, and fill; all
structures, pavement, and contours within 100 feet of wetland boundaries; all alterations
within the wetland resource areas; and all dates of fieldwork.
32. The owner of the property described in this Order must advise any potential buyer of the
property that any construction or alteration to said property, including brush cutting or
clearance, may require approval by the Northampton Conservation Commission.
Any instrument conveying any or all of the owners' interest in said property or any
portion thereof, shall contain language similar to the following:
"This property is subject to the Northampton Wetlands Protection
Ordinance and/or Wetlands Protection Act. Any construction or
maintenance work performed on this property requires an Order of
Conditions, and/or a Determination of Applicability from the Northampton
Conservation Commission.

19. Prior to the initiation of any work, the applicant/owner shall submit a letter of
understanding to the Commission stating that he/she has received, read, understands and
shall comply with these Orders. The applicant, and, his or her contractor, foreman and/or
construction inanager shall sign the letter of understanding.
20• Prior to the initiation of any ��ork, the applicant shall submit to the Commission a
sequencing plan for construction, vrd erosion and sedimentation control installation.
21. Prior to the start of any site work, excavation or construction, a pre-construction
conference shall be held on the site, between the contractor conducting the work, the
site/project engineer, the applicant, and a member or agent of the Conservation
Commission, in order to ensure that the requirements of this Order are understood by all
parties. Prior to the pre-construction meeting,.all erosion control devices must be
installed.
22. All required permits must be obtained from the Army Corp of Engineers (Section 404,
Water Quality Certification), Planning Board, Zoning Board, Department of Public
Works, or Building Inspector prior to the start of any project.
23. A copy of this Order and associated plans shall remain on site during all construction
and/or building activities. The project manager and all equipment operators shall be
familiar with the approved plans, and shall be informed of their location on the site.
This location shall be accessible to all contractors whenever work is occurring on
site.
24. All revised plans, referenced within this Order of Conditions, shall be approved by the
Conservation Commission and incorporated into the permit by reference and shall be
followed during the course of constriction.
25. The areas of construction shall remain in a stable condition at the close of each
construction day. Erosion control measures shall be inspected at this time, and
maintained or reinforced as necessary. All such devices shall be inspected, cleaned or
replaced during construction and shall remain in place until such time as stabilization of
all areas that may impact resource areas is permanent. These devices shall also be
inspected to assure that the maximum control has been provided. Any entrapped silt shall
be removed to an area outside the buffer zone and resource areas, and maintained or
reinforced as necessary. Erosion controls shall be inspected after every rainfall to assure
that maximum control has been provided.
26. An adequate stockpile of erosion control materials shall be on site at all times for
emergency or routine replacement and shall include materials to repair or replace silt
fences, straw bales, erosion control blankets, riprap, filter berms or other devices planned
for use during construction.

C. General Conditions Under Massachusetts Wetlands Protection Act
10. Where the Department of Environmental Protection is requested to issue a Superseding Order, the
Conservation Commission shall be a party to all agency proceedings and hearings before DEP.
11. Upon completion of the work described herein, the applicant shall submit a Request for Certificate of
Compliance (WPA Form 8A) to the Conservation Commission.
12. The work shall conform to the plans and special conditions referenced in this order.
13. Any change to the plans identified in Condition #12 above shall require the applicant to inquire of the
Conservation Commission in writing whether the change is significant enough to require the filing of a
new Notice of Intent.
14. The Agent or members of the Conservation Commission and the Department of Environmental
Protection shall have the right to enter and inspect the area subject to this Order at reasonable hours
to evaluate compliance with the conditions stated in this Order, and may require the submittal of any
data deemed necessary by the Conservation Commission or Department for that evaluation.
15. This Order of Conditions shall apply to any successor in interest or successor in control of the
property subject to this Order and to any contractor or other person performing work conditioned by
this Order.
16. Prior to the start of work, and if the.project involves work adjacent.to a Bordering Vegetated Wetland,
the boundary of the wetland in the vicinity of the proposed work area shall be marked by wooden
stakes or flagging. Once in place, the wetland boundary markers shall be maintained until a Certificate
of Compliance has been issued by the Conservation Commission.
17. All sedimentation barriers shall be maintained in good repair until all disturbed areas have been fully
stabilized with vegetation or other means. At no time shall sediments be deposited in a wetland or
water body. During construction, the applicant or his/her designee shall inspect the erosion controls
on a daily basis and shall remove accumulated sediments as needed. The applicant shall immediately
control any erosion problems that occur at the site and shall also immediately notify the Conservation
Commission, which reserves the right to require additional erosion and/or damage prevention controls
it may deem necessary. Sedimentation barriers shall serve as the limit of work unless another limit of
work line has been approved by this Order.
18. All work associated with this Order is required to comply with the Massachusetts Stormwater Policy
Standards,

C. General Conditions Under Massachusetts Wetlands Protection Act
(only applicable to approved projects)
1. Failure to comply with all conditions stated herein, and with all related statutes and other regulatory
measures, shall be deemed cause to revoke or modify this Order.
2. The Order does not grant any property rights or any exclusive privileges; it does not authorize any
injury to private property or invasion of private rights.
3. This Order does not relieve the permittee or any other person of the necessity of complying with all
other applicable federal, state, or local statutes, ordinances, bylaws, or regulations.
4. The work authorized hereunder shall be completed within three years from the date of this Order
unless either of the following apply:
a. the work is a maintenance dredging project as provided for in the Act; or
b. the time for completion has been extended to a specified date more than three years, but less
than five years, from the date of issuance. If this Order is intended to be valid for more than three
years, the extension date and the special circumstances warranting the extended time period are
set forth as a special condition in this Order.
5. This Order may be extended by the issuing authority for one or more periods of up to three years each
upon application to the issuing authority at least 30 days prior to the expiration date of the Order.
6. Any fill used in connection with this project shall be clean fill. Any fill shall contain no trash, refuse,
rubbish, or debris, including but not limited to lumber, bricks, plaster, wire, lath, paper, cardboard,
pipe, tires, ashes, refrigerators, motor vehicles, or parts of any of the foregoing.
7. This Order is not final until all administrative appeal periods from this Order have elapsed, or if such
an appeal has been taken, until all proceedings before the Department have been completed.
8. No work shall be undertaken until the Order has become final and then has been recorded in the
Registry of Deeds or the Land Court for the district in which the land is located, within the chain of title
of the affected property. In the case of recorded land, the Final Order shall also be noted in the
Registry's Grantor Index under the name of the owner of the land upon which the proposed work is to
be done. In the case of the registered land, the Final Order shall also be noted on the Land Court
Certificate of Title of the owner of the land upon which the proposed work is done. The recording
information shall be submitted to this Conservation Commission on the form at the end of this Order,
which form must be stamped by the Registry of Deeds, prior to the commencement of work.

MECHANICAL SYSTEM: The HVAC system shall provide
heating, cooling and ventilation compliant with 780 CMR 28, 780
CMR 13 and ASHRAE standards. The system shall consist of three
(3) exterior roof top mounted gas fired/electric cooling units
ducted thru the roof of the addition and ducted throughout the
NARRATIVE: The proposed Addition (F-1 use
group) addition shall be constructed of a pre-engineered steel
building system (Type 2-C) , with interior partitions constructed
of steel studs and gypsum wall board. The entire addition shall
be have an automatic sprinkler system by extending the existing
municipal water main in the street into the Addition and
extending the fire protection system into the existing
manufacturing building. The setbacks from other buildings is
greater than 10, therefore the exterior walls are not fire
rated. A common wall between the existing manufacturing building
and a earlier addition is a concrete masonry wall and is rated
at 2 hours. The Addition will not be fire separated from the
existing building by a fire rated wall .
CHAPTER 34 NARRATIVE: The construction of the addition
will result in a Hazard Index of 3, which is the same HI as the
use of the existing manufacturing building. The existing
manufacturing buildings have been constructed within the past 15
years therefore it is assumed the construction is in compliance
with 780 CMR 6th Edition.
FIRE PROTECTION SYSTEMS: The existing building shall be
equipped throughout with an automatic sprinkler system. The
sprinkler system will originate in the Addition and extend into
the existing manufacturing building. Design of the automatic
fire sprinkler system will conform to NFPA 13, "Installation of
Sprinkler Systems" . The sprinkler system will be monitored by a
third party monitoring company.
All means of egress exits shall be marked with lighted "exit"
signs and lighted with battery back-up emergency lighting, with
exterior remote light fixtures. Emergency pull boxes will be
