BUILDING PROCEDURAL FORM1
This Building Procedural Form is designed to give notice to each
appropriate City Department, having jurisdiction by Law, Code
and/or Regulation, of the erection of a proposed building or
development. The purpose of this procedure is also to give,
notice to the Applicant as to what permits, approvals, etc. , it
will be necessary to obtain.
This form and the signatures from the various Departments in no
way indicates approval, but is intended as a means of . assistance.
Individual approval by each of the appropriate Departments is a
separate action above and beyond the scope of the procedure. All
determinations made in this procedure shall be based upon the
information submitted by the Applicant.
The Builder or Owner shall have the responsibility of obtaining
the information required on this kform, and returning the
completed form to the Building Inspector prior to the issuance of
a Building Permit.
Disapproval of a project by a City Department other than the
Building Inspector is not necessarily grounds for the with-
holding of a Building Permit. However, no project may legally
commence until all of the required permits and approvals have
been obtained. The Building Inspector may, at his discretion,
exempt projects from this process which are limited to: demoli-
tion; interior alterations of single or two-family dwellings
which do not involve municipal utilities; or exterior alterations
which do not increase the floor area of the structure.
The Applicant shall fill out the form to the double line only.
The respective City Departments will complete the portion of the
form as designated. In addition to the form, the Applicant shall
also submit a Site Plan that should 'include and accurately
depict, where applicable, the following (the extensiveness of the
Site Plan shall be commensurate with the extensiveness and scale
of the project) :
- drawn to an engineering scale of 1" = 40 '
- location of all structures (both existing- and proposed)
- location of driveways
- location of parking areas
- location of water bodies, wetlands and floodplains
- location and elevations of sanitary sewer and water lines
(gate valve sizes, materials, etc. )
- location and elevation of sewer and water tie-ins (type,
size, materials, etc. )
- dimensions/lot size, frontage, setbacks of structures
- topography (contour elevations commensurate with scale of
project)

1) I will contact the Department of Public Works and have
an inspector check and approve the graded gravel base
prior to paving to insure compliance with slope and
location;
Z) I further agree that if in the inspections any of the
permit conditions are not met that I will at no expense
to the City remove and replace the driveway as directed
by the City Engineer.
