11-002 Weekly Activity Report Summary WEEKLY ACTIVITY REPORT SUMMARY VAMC Northampton, MA Contract # VA241-C-0125 Week of: June 1-4, 2010 Site Superintendent: Pay Mahan QA/QC Manager: Mark Campbell, PE AES Project Manager: Philip Smith, P.G. Operations: Completed excavating basins 1 and 2 to subgrade. Fill removed from basin areas moved to upper landfill area. Test pits performed at basin 1 to confirm bottom. Removal from site of all stumps in progress. Fill installed in the bottom of both basins. Clay berm being constructed at basin 1, approximately 75% complete. Bates placing clay in 6” lifts and compacting. Yankee Testing on site 6/4 to perform compaction tests on clay. Both tests passed over 95%. Performed silt fence maintenance. Bates confirmed top of berm elevation for QA/QC representative. Materials Purchased/Delivered/Installed: multiple loads of clay, common fill, sand and loam. Operations: Bates completed installation of sand layer under geo-membrane on Wednesday. Also installed gas vent piping. Membrane liner started and completed on Thursday. Geo-textile material installation commenced on Friday Materials Purchased/Delivered/Installed: Multiple loads of sand Subcontractor Work: New England Liner installing liner and geo-textile Equipment on site: See report for last week. SWPPP Inspections: No rain event this week. Hay bales and silt fence inspected and found to be in working condition. Bates rolled and cleaned up access road Safety Violations: None MA DEP/City of Northampton Personnel Site Visits: Charlie Clines, DEP. Charlie noted build up of stone along silt fence on access road. I advised him that the situation has been stable for several weeks, and we will continue to keep and eye on it as well as clean up the access road. Overall, Charlie was pleased with the progress and quality of the work. Comments: Light sprinkle on Wednesday allowed Bates to achieve compaction of the sand without st use of water truck. All 8 tests passed 1 try. Liner installation QC; observed all air testing and vacuum testing. Destructive testing in site truck passed. Samples sent out to independent lab. Operations: Having completed the work of establishing the limits of the new landfill, Bates is now focusing operation on rough grading the site to final landfill contours. Bates established a haul road going up the middle of the landfill to allow for ease of delivery of the multiple loads of common fill needed for the last layer of daily cover. Rough grading being accomplished mostly with the use of the D6 dozer, followed up by the vibratory roller. Prior to installation of final layer of daily cover, I walked the site with Pat to identify items that may be of concern, i.e. large chunks of concrete, etc. that need to get buried deeper to prevent popping up over time. Materials Purchased/Delivered/Installed: Multiple loads of sand to be used as common fill. Geonet material (24 rolls) along with test certs. Operations: Bates completed installation of common fill layer on Friday. Also dug for anchor trench. Materials Purchased/Delivered/Installed: Multiple loads of sand to be used as common fill. Subcontractor Work: None Equipment on site: See report for last week. SWPPP Inspections: SWPPP based inspections have been occurring since I arrived on the project on st 6/7/10. As per the SWPPP plan in the contract documents, Bates 1 activity, after installation of all hay bales and silt fence, was to construct the retention basins. Bates began that work on 6/1/10 and completed it on 6/10/10. There were several rain events during the construction of the basins, but the clay barrier prevented any runoff from the basin construction. Additionally, the site had been cleared and grubbed, but there remained a firm top soil level such that runoff from the landfill site itself was essentially zero. No discharge of pollutants was observed to leave the site. Upon completion of the basins, an unusually dry period occurred, in fact it is still occurring. There have been no significant rain events since the completion of the basins to this current date. Hay bale and silt fence inspections continue on a weekly basis. The access road, which is covered in crushed stone, has stone build up along the sides of the silt fence, but the situation has been stable for 3-4 weeks. Bates brought in a water truck on Friday, moistened and re-rolled the site, and the required 8 compaction tests all passed. I also had Bates did eight test pits in the common fill to confirm proper depth. All were within a ¼”, most were somewhat more than the required 12”.
